Forms for Minors: There are two forms for minor (under 18 years old) Boston Harbor attendees. The Health History and Medical Record is to be filled out and signed by the parent (or legal guardian) and returned by July 1st for EVERY Minor School Participant.
The Minor Release Form is to be filled out for each Minor School Participant who is not being accompanied to the Boston Harbor Scottish Fiddle School by a Parent/Legal Guardian. It is also to be signed by the minor attendee and by the designated non-parental Guardian, and returned by July 1st.
